Fall 2005 Season: Adult Six-A-Side Registration Monday, 24 October – Monday, 12 December, 2005 Ø Games played on P.F.C. fields with F.I.F.A.-sanctioned Referees Ø Games played on Mondays (some Wednesdays). Always check web site. Ø New players must pay for a league jersey (one-time $15 fee) & pose for player pass I.D. card photo (taken by the league at no charge) Ø Must have at least turned 17 within 2005 FEES (*Once the season starts, the league does not offer refunds.) EARLY Registration NOW through Friday, 21 October $60 Late Registration Saturday, 22 October $80 AFTER the first game (Monday, 24 October) $100* Jersey (one-time, required & provided by the league) $15

Register NOW! PLEASE, do NOT wait until opening game night. Your Board Members, who register you, want to play in their games too. Also, we need accurate information to create the divisions & game schedule BEFORE opening night. There will be NO registrations on opening night. No exceptions!!!

This is a 2-Part Process: 1. Fill out the form. 2. Then, you MUST make your payment by hitting the "Submit" button when you are finished filling out the form. If you do not do so, you are NOT registered. There are NO spots held. In other words, 1st to pay = 1st to play. You may pay using Visa, MasterCard, Discover or American Express.
*Remember, if you do not immediately make your payment online...you are NOT registered. Your registration form will simply be thrown away. PLEASE pay at the same time!!! Can we make it any clearer?!?

2. Fill out the form entirely. PLEASE print legibly! If we can't read it, we can't register you!
3. Then, you must attach your payment by cheque. Make it out to "Paso Adult Soccer Club". If you do not do so, you are NOT registered. There are NO spots held. In other words, 1st to pay = 1st to play.
4. Send your completed form (the last page of the download) AND CHEQUE to:
